Injuries and Concussion Protocol
FM27 adds a concussion protocol, built with the Professional Footballers' Association, under which a concussed player needs a neurologist's clearance before he can play again, tracked in the medical screens; SI has named no other change to injuries.
Football Manager 27's one announced change to injuries is a concussion protocol: built with the Professional Footballers' Association, it means a concussed player needs a neurologist's clearance before he can return, and his progress is tracked in the game's medical screens, which SI calls the "Medical Centre." Sports Interactive has described no other change to how injuries happen or heal as of Nov 1, 2026, so the rest is presumed to work as in Football Manager 26. This comes from SI's deep dive of Oct 16, 2026, on a development build, before the Nov 10, 2026 release. In the clip Zealand (Oct 21, 2026) runs through SI's notes and hopes concussions have not been made more frequent.

How does the concussion protocol work in FM27?
When a player suffers a concussion, he cannot simply be picked again once he feels fine. He must be cleared by a neurologist before he returns to selection, and the medical screens track the case. SI has not said how long clearance takes, whether it differs by competition, or whether the protocol can be switched off.
The medical screens and injury updates
The medical screens hold the squad's injuries and risks. FM27 adds more tooltips, including on medical reports. In SI's official screenshot of a Brighton save in December, the inbox carries a "Brighton injury risk update" and several fitness updates from the staff, each tagged Injuries, so injury news arrives in messages as before. The screenshot is a development build. The clip is WorkTheSpace's Oct 30, 2026 beginner's guide, on the actions an injury message offers.
Stamina, fatigue and the match engine
The match engine's stamina model changed: stamina now affects ground covered, repeated sprints and recovery, and condition over 90 minutes, so substitutes matter more. Training shows player load for the coming weeks, not just the next fixture. SI has not tied either change to injury rates. More on Match Engine and Training. The clip is SI's Oct 5, 2026 deep dive, over development-build footage.
What is not known
As of Nov 1, 2026: whether injury frequency, recovery times or the risk model changed, and whether the protocol is the same on FM27 Console, FM27 Mobile and FM27 Touch.
